· Develop personalized support plans tailored to the needs of each client.
· Work collaboratively with other professionals and community organizations.
· Maintain accurate records and reports as required.

We are seeking a compassionate, proactive, and resilient individual who:
· Has excellent communication and interpersonal skills.
· Is empathetic and non-judgmental, with a passion for supporting vulnerable individuals.
· Possesses a strong understanding of safeguarding and confidentiality principles.
· Is able to work flexibly and adapt to the changing needs of clients.
· Holds a valid driving licence and access to a reliable vehicle (preferred).
· Has a background in social care, healthcare, or a related field (beneficial but not essential).
