Summary
Family Welfare Coach
Part time (29.6 hours) flexible hours
Salary: SP9 £24,319 – SP13 £26,012
pro rata (dependent on experience & qualifications)
Permanent subject to funding
KEY TERMS & CONDITIONS:
• 29.6 hours per week, 25 days holiday pro rata, 1-day birthday leave, 8% non-contributory pension.
• Cycle to Work scheme – Buy a bike tax free and cycle to work.
• Westfield Foresight Health Plan – Annual benefits and services such as routine dental work and check-ups, etc.
The Family Welfare Coach supports families facing complex challenges by building strong, trusting relationships and walking alongside them to improve wellbeing, confidence, and independence. Using a trauma-informed and culturally responsive approach, you will offer holistic coaching support, help families navigate services, and connect them to social prescribing pathways and community-led solutions. The role will:
• Co-ordinate, simplify and broker access to a range of health and social interventions based on the needs and wishes of the individual, including liaising with a range of agencies (schools, housing, Family Intervention Service - FIS)
• Empower the individual to express their views, wishes and concerns
• Co-produce a personalised support plan that encourages the empowerment of an individual to take control of their own health and wellbeing (‘what matters to me’)
• Facilitate delivery of a peer led ‘mindful mamas’ group
• Connect individuals to local and city-wide services
• Develop close working partnerships with health, social care, housing and voluntary, community or social enterprise (VCSE) organisations.
An Enhanced DBS is required for this role.
