Our client is seeking a compassionate and proactive Caseworker to join their client's dedicated Family Support Service team in North/South London. In this rewarding role, you will manage a team of support workers, ensuring that personalised care is delivered to help service users lead as independent a life as possible.
As a Caseworker, your responsibilities will include accepting referrals, arranging home visits, and conducting assessments to develop bespoke support plans tailored to each individual's needs. Your role will be vital in helping service users achieve their personal goals while maintaining their dignity, privacy, and independence.

They are a well-established Domiciliary Care and Support service provider in South London with over 15 years of experience. They specialise in providing high-quality, person-centred care services for children, young people, and adults with learning and/or physical disabilities. Ideal Candidate:
- NVQ Level 3 in Health and Social Care or equivalent qualification.
- Strong communication skills, both written and verbal.
- Experience in a similar role within domiciliary care.
- Excellent organisational and problem-solving abilities.
- A strong understanding of person-centred care principles.
- Proficiency in IT systems for scheduling, reporting, and communication.
- Flexibility to work evenings and weekends as needed.
Desirable requirements and skills
- Empathetic nature to adequately understand clients' predicaments and provide the necessary support.
- Experience of attending and presenting in case meetings.
- A Full UK or International Driving Licence.
Key Responsibilities:
- Deliver person-centred support services tailored to the diverse needs of service users.
- Conduct assessments and develop individualised support plans that reflect clients' goals.
- Support service users to set clear goals and take action toward achieving them.
- Promote access to education, training, employment, and life skills to build independence.
- Collaborate with external agencies and professionals to deliver coordinated support.
- Supervise support workers and provide guidance to ensure high standards of care.
- Promote active service user involvement and effective communication across all care activities.
- Maintain accurate and timely records in line with safeguarding and data protection standards.
- Provide support during crisis situations, with flexibility to work outside of normal hours if needed.
