Summary
An exciting opportunity has arisen for a Band 4 Patient Pathway Co-ordinator which focuses on supporting the nationwide Target Lung Health Check Programme. The successful candidate will join the Respiratory team at Southend University Hospital, Mid and South Essex NHS Foundation Trust. The role includes working 3 days a week. This co-ordinator post is a crucial role in the success of the Target Lung Heath Check Programme particularly when the focus includes early detection of lung cancer.
If successful you will use your skills and expertise to provide efficient and effective support. Having a positive, enthusiastic and "can do" attitude with a strong work ethic is important. You must have excellent communication skills and be comfortable in dealing with patient's telephone calls and queries. You will require competency in all Microsoft Office 365 packages specifically excel. The ability to demonstrate your initiative and work as part of a dedicated team is an important part of this role.
Programme co-ordination overseeing the scheduling of the programme, ensuring smooth patient flow through each stage, from initial screening to follow up ensuring further diagnostic testing is promptly referred and ongoing monitoring of the patient pathway is tracked efficiently.
Collaborating closely with multidiscliplinary teams including radiologists, respiratory specialists and other healthcare providers to co-ordinate care
Data management.
Record keeping. Maintaining accurate patient records, including results of screenings, referrals and follow up care.
Reporting: compiling and analysing data to measure the programme's success, including the number of individuals screened, diagnosis rates, treatment outcomes.
