Summary

Phoenix Future's Derbyshire Substance Use Service is looking to recruit a highly motivated, passionate, person-centred and recovery focused individual to join their team.

The specialist Tier 4 Substance Misuse Practitioner role will involve working with service users within the Derbyshire county who are already accessing structured substance misuse treatment within the service, and are now working towards accessing funded Tier 4 inpatient detoxification and rehabilitation.

As a Tier 4 worker you will deliver structured and intensive 1:1 support alongside the service user's allocated key worker, and support the facilitation of groups. You will liaise with key workers, detox/rehab providers, Commissioners and other involved care professionals in supporting service users with achieving their Tier 4 treatment goals.

This role will be a full-time 37.5 hours per week contract, working Monday-Friday 9am-5pm. Base is flexible - you can request to be based at any one of our four sites within the Derbyshire county - our main bases are in Chesterfield, Ripley, Ilkeston and Swadlincote. This role is for an initially fixed term contract to 31/03/26.
