Summary

PRINCIPAL DUTIES & AREAS OF RESPONSIBILITY Acts in accordance with The Leeds Way at all times. Act in such a way that safeguards the health and wellbeing of children and vulnerable adults at all times. Is familiar with and adheres to, the LTHT safeguarding policies. Raise concerns with regards to risk, danger, malpractice or wrong doing by following the LTHT Whistleblowing policy and supporting processes. Undertake mandatory and priority training within the required time scales Comply with the wearing of PPE as appropriate to the clinical situation - this may involve wearing face masks for a considerable length of time Engages in the Leeds Improvement Method to improve the quality and safety of our services. Responsibilities for patient and client care - Assesses, plans, implements, and evaluates programmes of practical and emotional support, under the supervision of the Senior Clinical Nurse Specialist or a designated deputy. - Organises own client-based workload. Freedom to act - Refer patients and families as appropriate to other members of the multi-disciplinary team - Able to act independently and competently when making decisions within the remit of their job roles. - Make decisions regarding support-based assessments and interventions within clearly defined policies and protocols. - Work as a team member as well as on own Communication - Maintains accurate records using the Leeds Teaching Hospitals Trusts documentation. - Establish and maintain effective communication with patients, significant others and patient groups. - Demonstrate a variety of communication skills with patients who may have communication difficulties e.g. or other linguistic issues. - Demonstrate good communication skills in difficult circumstances. - Support patients and their families communicating sensitive condition related information to the relevant clinical teams. - Liaise with Community and Charity Services (as appropriate) to ensure seamless provision of care delivery. Education and Training - Provide training and education of learners and other clinical staff where appropriate. - Actively participate in the development of own personal development plan and performance review - Keep up to date with developments, advances within the specialty where appropriate. Leadership and management - Engages users and carers views on service delivery and development. - Contribute to policy and protocol development for speciality area. - Exercises a personal duty of care in relation to equipment, resource and stock control utilising equipment and supplies appropriately. - The postholder will undertakes any surveys or audits as requested or appropriate in own area of work.
