Summary
Communication and relationship skills Provide, receive and understand sensitive and highly complex information relating to children and young people with complex health needs, and be competent in providing specialist advice and support on particular clinical issues relating to their conditions. Communicate sensitive information effectively relating to medical diagnosis and children with additional needs with partners both within the NHS and those external to it, including the voluntary sector, in order to develop and promote collaborative working. Communicate information in an empathetic and sensitive way to parents/ carers which relates to the clinical care of their child/young person. Be able to identify when to share relevant information appropriately. Demonstrate proficient management of barriers to understanding such as learning disabilities, age and cognitive abilities and utilise specialist knowledge around child development to overcome these. Provide both written and verbal information which is clear, concise and understood. Able to manage contentious issues sensitively. Confident in working with other providers and professionals including in primary care, social care, voluntary sector, acute hospital trusts and allied health professionals. Knowledge, training and experience Registered Childrens Nurse with considerable clinical experience. Community experience desirable. Mentorship qualification. Sound ability to assess, plan and evaluate evidence-based nursing care. Provide expert assessment, diagnosis, and management of children and young people with asthma in the home, clinic, or school setting. Develop and review personalised asthma action plans in collaboration with families and healthcare professionals. Lead on the implementation of national asthma care standards and NICE guidance. Evidence of appropriate clinical competence and ongoing professional development. Maintain clinical and professional competency. Experience of managing own caseload. Understanding of community setting for care integrated working with wider MDT. Promote patient and public involvement in activities designed to inform service improvement. Actively participate in clinical audit and service improvement activity to improve patient care and patient outcomes (privacy, dignity, and duty of candour). Analytical and judgement skills Ability to prioritise and respond appropriately to patient referrals. Understand complex information and provide solutions Proactive management of risks. Maintain and actively seek an awareness of national information and be actively involved in local implementation in order to improve service delivery. Recognise own limitations and be aware of when to sign post; sharing relevant information. Planning and organisational skill Plan and organise own clinical workload within a specific locality and be able to work effectively with an often unpredictable, work pattern. Effective time management of work to meet the demands of the service. Ability to adapt and be flexible in responding to changing needs and demands of the service. Create, plan, organise and deliver educational packages to patients, relatives, carers and other health professionals in relation to a specific condition and ongoing clinical need. Attend all Team meetings. Physical skills Undertaking advanced clinical skills required by caseload. Good hand-eye coordination and accuracy, safe use and understanding of medical device equipment relating to the child/young persons needs. Good IT skills to communicate with other members of the MDT, record notes on electronic patient records system, type letters and reports. Responsibility for patient / client care Manage a large and diverse patient caseload and provide high quality clinical advice, information and support to children, young people and their families. Ability to assess and develop specialist care plans for children/young people accessing the service and implement and subsequently evaluate individualised care plans Provide on-going support for parents and carers in the home or community setting to maintain treatment plans. Ensure patient safety and safe patient handling, according to local policies, at all times. Ensure specific training is received and remains up to date for the use of all medical devices. High quality record-keeping and in a timely manner. Sound safeguarding children and adults knowledge. Provide ongoing telephone advice to children, young people and their families to ensure needs are met between visits, as required. Responsibility for policy and service development implementation Be aware of and working within all MCH policies, procedures and guidelines and maintaining compliance with all mandatory updates. Lead on specialist service development including developing specific local guidelines and protocols, and competencies. Participate in initiatives and developments which facilitate the advancement of nursing practice ensuring continued improvement to patient care. Awareness of relevant partners policies, procedures and guidelines such as social care and education, and recognition and appreciation of the need for joint working in certain situations. Responsibilities for financial and physical resources Assisting with maintaining stock of clinical equipment such as CVAD equipment, dressings, other ancillaries etc, which includes directing the ordering of such resources. Be aware of the service budget and actively work with the Clinical Lead to reduce spend where possible. Responsible for safe and careful use of all medical device equipment and specialist supplies. Responsible for the safe and careful handling of personal possessions of patients, including equipment purchased by individual families. Responsibilities for human resources (HR) Provide and contribute to the learning environment for students/learners of multi disciplines undertaking clinical placements within the team. Promote a positive learning environment and contribute to and develop a meaningful educational experience for learners from other disciplines through devising and facilitating teaching programmes and in-house training. Attend supervision as directed. Support and work alongside the senior team members with regards to operational issues of the service, in order to ensure efficient day to day organization of the childrens community nursing service and to ensure a safe and effective service is delivered to the children/young people. Responsibilities for information resources Responsible for accurately updating patient held notes Able to demonstrate IT skills (eg- Word, MS teams, EPR systems) Responsible for safe and proper use of patient identifiable information according to local patient confidentiality and record keeping policies and within NMC guidelines. Complete and return all individual reports/statistical information required by the Head of Service within the required timeframe. Maintain electronic diary in relation to daily activities in order to comply with lone working.
