Summary
• The post holder will provide advice and support to Staff within TowerHamlets on the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care.
• Be responsible forundertaking and quality assuring NHS Continuing Healthcare Assessments with members of the Multidisciplinary Team(MDT), ensuring that cases are presented to panel within the specifiedtimeframe.
• Undertake reviews for NHS Continuing Healthcare and NHS FundedNursing Care Patients and ensuring that they are completed withina timelymanner.
• To case manage Patients who meet the criteria for NHSContinuing Healthcare.
• To arrangeCare Packages/Placements for Patients deemed eligible forNHS Continuing Healthcare in conjunction with Commissioning. To complete Personal Health Budget Care Plans in collaboration with Patient, Family and MDT. To provide ongoing monitoring to en

• To agree a shared care plan with members ofthe Multidisciplinary Team for Patients who are assessed as meeting the criteria for shared care with the Local Authority, ensuring robust care plans are in place to support thedecision.
• To case manage Patients who meet the criteriafor NHS Continuing Healthcare. Tomonitorthe standard of care provided and coordinate, liaise with Health and Social CareProfessionals.
• To provide advice and support to the MDTregardingNHS Continuing Healthcare Assessments, provide quality assurance onbehalf of the Clinical Commissioning Group (CCG) and ensure that the Patient’s case is discussed at Continuing Healthcare Panel within28 daysof receipt ofreferral.

• Toparticipatein any Disciplinary or GrievanceProcedure.
• To support the Lead Nurse byparticipatinginformulating,updatingand reviewing all policies and procedures asappropriateand relevant to the area ofpractice.
• To ensure that trust policies and procedures are maintained at all times,confidentiallybeingparamount.
• Toassistin other areas as required by Lead Nurse. Thismayinclude occasionally working outside of your current role and in different worklocations.
