Summary
Provide clinical input on medicines related issues, quality improvement, supporting aspects of the quality and outcomes framework and locally commissioned services. Support repeat/acute prescription requests, medicines reconciliation on transfer of care and systems for safer prescribing. Provide face to face patient clinics. Ensure patients are referred to the appropriate health care professional for the appropriate care linking in with social prescribing models to support broader health and well-being needs. Support integration with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage the practice workload. The specific tasks will be agreed and identified by the practice. You will be part of the practice team but also provided with peer support and education via the South Manchester Pharmacists and Technicians across 22 practices.
