Summary


We are pleased to offer an exciting opportunity for a Band 4 professional to join our dedicated diabetes team, working alongside consultants, specialist nurses, and dietitians.

We’re looking for a highly motivated individual with a passion for developing their skills, particularly in patient support and education around self-management of diabetes.

In this role, you will play a key part in delivering education and advice to patients, families, carers, and other healthcare professionals.

The ideal candidate will have a strong interest in diabetes care, a proactive approach to learning, and a commitment to delivering high-quality education and support to help improve patient outcomes.
• Providing one to one education and structured group education to patients with diabetes. Coordinating group education sessions
• Supporting patients with the initiation of diabetes technology such as continuous glucose monitoring systems.
• Supporting consultants in clinics
• Visiting patients at home to assess and support them with their diabetes
