Responsibilities
Assess clients' physical, emotional, and social needs to develop tailored care plans.

Provide education and resources to clients and families regarding available services and support systems.
Maintain accurate and up-to-date documentation of client information and care coordination activities.
Assist clients in accessing community resources, including healthcare services, transportation, and support groups.
Conduct regular follow-ups to evaluate the effectiveness of care plans and client satisfaction.
Requirements
Bachelor's degree in social work, nursing, or a related field.
Previous experience in care coordination, case management, or a healthcare-related role is preferred.
Strong organizational and time management skills to handle multiple clients and priorities.
Excellent communication and interpersonal skills to work effectively with clients and team members.
Ability to assess needs and develop comprehensive service plans for diverse populations.
Proficiency in using electronic health records and other relevant software applications.
Knowledge of community resources and healthcare services available to clients.
