Summary
The role is based in co-located offices with our colleagues from health, either at the Princess Royal University Hospital (PRUH) or the Single Point of Access (SPA) offices currently in Orpington. One Bromley - working together - South East London CCG (selondonccg.nhs.uk) The post involves sourcing domcare support packages to enable people to go home from hospital where they have health and social care needs. You will be required to work closely with our Colleagues from the PRUH and SPA to enable discharge from any hospital in and out of borough where the London Borough of Bromley have a duty to meet needs. This role will involve weekend working and as an example would cover: Week 1: Monday, Tuesday, Saturday and Sunday. Week 2: Tuesday, Wednesday, Thursday.
This is a full time role on a temporary contract basis.
