Summary
Provide a holistic approach to patients, including assessment, diagnosis, planning and implementation of high-quality care within chronic disease scope of practice.
To work collaboratively within the team to meet the needs of patients using evidence based care principals. Liaising with the Nursing & AHP Clinical lead, responsible for ensuring required audits are completed and all practice staff comply with local and national policy.

The Senior Practice Nurse will work as part of a multi-disciplinary team, responding to referrals from GPs, other community nurses and health & social care colleagues.
This post requires extensive autonomous practice. The post holder shall provide independent, critical thinking and diagnostic reasoning when making clinical decisions within chronic disease management scope of practice.
The post requires high level clinical expertise within chronic disease management and a relevant clinical skills.
It is essential that you are able to meet the transport requirements of the post.

Northumbria Primary Care Ltd (NPC) is a pioneering partnership with local GPs and now manages several GP practices with around 120,000 registered patients in Northumberland and North Tyneside.
Whilst appointees will have a base practice within NPC, they may be asked to work at other practices across our group. We do however limit this as much as possible as we appreciate continuity is vital to effective primary care.

• To work as the Nursing Lead within the Practice, providing senior leadership and support to other members of the Nursing and Health Care Assistant Team.
• To provide a needs led focused service dictated by the Practice Strategy and the framework of the NHS modernisation agenda.
• To be accountable for ensuring that agreed professional standards are reflected in clinical practice supporting the delivery of policies and procedures.
• Work autonomously and operate as lead nurse for Chronic Disease Management and deliver a high standard of advanced clinical skills by through obtaining a formal clinical skills and nonmedical prescribing qualification. This shall support the provision of enhanced nursing services to the whole practice population including chronic disease management, health promotion, maintaining immunisation and screening programmes.

• To operate as Infection Control Lead for the Practice. Liaising with the Nursing & AHP Clinical lead, responsible for ensuring required audits are completed and all practice staff comply with local and national policy.
• To lead on and support the junior team in promoting an environment conducive to the training needs of other staff and students and participate in education programmes as required.
• The post holder frequently will work unsupervised and is accountable for care delivered, demonstrating critical thinking and skills in decision making in the management of patients.
• The post holder shall assist the Nursing & AHP Clinical Lead and Practice Manager in clinical supervision and appraisals of the junior nursing and healthcare assistant team.
• To role model compassionate and inclusive leadership in order to shape the creation of a collective leadership culture within the service. This means demonstrating a consistent leadership style which (a) engages, enables and empowers others (b) uses coaching to
promote ownership of learning and quality improvement and (c) facilitates team working and collaboration within teams / departments and across organisational boundaries.
