Summary
Supporting the LTC consultant with coordination and management of patients with complex multi-morbidity, taking responsibility for managing a case-load Regular liaison with a range of consultants and teams across Trusts (including community health and mental health teams) and to support case coordination and management Following-up actions following desktop review Regular contact across a range of GP practices Effective follow-up and monitoring of outcomes Line management of an admin coordinator role Liaison and contact with patients and carers A commitment to equity, diversity and inclusion and an understanding of how these principles apply to this role.
