Summary
Basing own practice on evidence of good practice in diabetes and nursing care, leading evidence based improvements and enabling others to apply them in practice. Contribute to fortnightly teaching and learning in the practice. To work closely with the Integrated Diabetes Service and other members of the multidisciplinary team including lead consultants, GP's, nursing colleagues, other health care professionals, social services and voluntary sector services. To hold and manage an active caseload. To undertake patient assessment, develop, implement and evaluate plans of care for patients with diabetes To participate in nurse led clinics, outreach and community clinics and to undertake home visits as required Undertake clinical procedures and interventions commensurate with role, including initiating and adjusting insulin doses as indicated and recommended oral diabetic medication in accordance with national and local guidelines/practice Provide telephone support assessment and advice to people with diabetes on their management in accordance with national and local guidelines/practice
