Summary
The post holder will be required to travel throughout Nottinghamshire, Derbyshire, Leicestershire, Lincolnshire, Staffordshire. In order to visit patients, the agencies involved in their care. Attend shared care clinics and regional meetings Liaise with Education, Primary Health Care Teams, Social Services and the Voluntary Sector providing them with the knowledge in relation to the highly specialised programmes of care required by children and young people with immunology conditions or HIV. Ensure safeguarding issues within the children and young peoples immunology or HIV services are, managed in a timely manner, liaising with the Trust Safeguarding team as appropriate Organise and facilitate case conferences and reviews as required. Act as an advocate for the children, young people with immunology conditions or HIV and their parents/carers Work in partnership with children, young people and their parents / carers developing and facilitating Support groups / education awareness sessions, Satisfaction surveys, Service development
