Summary
Key responsibilities:- - Assessing clients health and social care needs who present in acute crisis. - co-ordinating effective gate keeping assessments when needed. - providing a range of bio/psycho/social interventions for service users and carers - planning for a robust, sustainable discharge as well as sufficient aftercare - Completing collaborative care and crisis care plans with clients and their carers. - Leading shifts, involving fair delegation of work to colleagues. - Maintaining professional registration and keeping abreast of relevant mental health policy and evidence based mental health practice.
