Summary

As a Diabetes Specialist Nurse within the Intermediate care service the post holder will be responsible for delivery of safe and effective care and support of patients living with Diabetes. Educational provision in the care of diabetes, both internally and externally to the Trust is equally an essential elementThere is an expectation to support the County wide model of care whilst holding a case load within a specific locality linking with community neighbourhoods. We are currently embarking on an exciting journey to integrate patient pathways with our acute colleagues and the post will require a willingness to proactively accept and contribute to change KEY RELATIONSHIPSNurse Consultant-Diabetes, Dieticians, GPs, Matrons, Endocrinologist, Acute Diabetes Specialist Nurses, District Nurses, Community Hospital Nurses, Company Representatives, Patients, Carers, Practice Nurses, Neighbourhood managers Planning and OrganisationPrioritise own work efficiently and effectively to ensure Trust targets are metAnalyticsMake judgement when dealing with enquires, to resolve problems for patients and staffTo use analytical and judgemental skills for assessing, interpreting and acting upon results where appropriateResponsibility for Patient / Client Care, Treatment & TherapyTo work independently as a specialist autonomous practitioner, managing own caseloadRespond to patients queries in a timely fashionUsing experience and acquired knowledge to advise clinical colleagues of standard speciality practice to avoid delay in patient careTo plan care, advise others regarding the management care and participate in case conferences/virtual clinics as necessaryTo develop and implement specialised programmes of careTo provide specialist clinical services for patients and carers and signpost to agencies where social prescribing can benefit patient pathwaysTo deliver structured education courses following required training achievementThe post holder will provide care that mirrors service specification requirementsManage an ongoing patient case load of people with Diabetes via nurse led clinics, face to face, digital or phone consultation, ward reviews in community hospital settings and domiciliary visits, working towards agreed standards and targets as determined by the Nurse Consultant for Diabetes and current best practiceTo undertake regular duty shifts which include triaging and responding to patient / GP surgery requirementsUndertake review of hypoglycaemia reports submitted to the service identifying and acting on any required action for patient care or opportunity to support in the education of staffTo assess, plan and document care on an individual basis, evaluate the need for clinical review with the relevant Medical Practitioner when appropriateTo provide written evaluation of assessments and clinical decisions based on the assessments to the appropriate Medical Practitioner and / or Matron responsible for the patients careTo use consultation skills to identify the physical, psychological and social needs of the patient and carer referring them when appropriate to the relevant multidisciplinary team member and / or social services, carers support service and voluntary organisations. Act as advocate for the patient ensuring the provision of appropriate information and support servicesTo initiate and monitor effects of insulin therapyTo assess patients to detect and manage medication side effect or adverse reactions by communicating with the relevant medical practitionerTo provide specific counselling support and sign posting to patients and their carerTo provide written information and advice on specific aspects of care, such as drug treatments, side effects and other treatment options to patients and carerSupport people in their home environment, remaining the key diabetes contact if admitted to hospital or permanent careTo co-ordinate care of patients between primary and secondary care settings acting as an expert resourceAs part of a multi professional team provide shared care interface for specialist care and support for people with particular needs related to diabetes for example pre pregnancy preparation and post pregnancy advice, renal disease and complex case managementTo maintain accurate records and documentation of workload, advice given and communication with other professionals with due regard to legal issues and confidentialityTo promote communication between all agencies involved in the care of people with diabetesTo plan, deliver and evaluate group for patients and carers undergoing initiation of injectable therapiesTo share expertise by example, by maintaining a resource of current material for professional and people with diabetesTo contribute towards the development of the Diabetes Specialist Nurse role in order to continuously improve the treatment and support of people with diabetes and their carers Policy, Service, Research & Development ResponsibilityBe responsible for updating own knowledge as and when necessaryFollow relevant Trust policiesBe involved, feedback and comment on quality improvement proposals/changesBe proactive and positively contribute to quality improvement proposals/changes to procedures, supporting clinicians to adopt such changes to their working practicesTo keep up to date with all relevant guidance, NHS targets and relevant policiesTo keep up to date with all relevant researchTo keep up to date with all relevant drug Summary of Product Characteristics changesTo contribute to projects in relation to the development of the Trusts Corporate Service Plan and organisational objectives as required/appropriateTo undertake research and development surveys, audit and evaluationTo contribute towards the development of guidelines and protocols, which reflect best practice and enhance programmes of careTo work with the Nurse Consultant for Diabetes, Service management team and senior DSNs to support and maintain a clinical managed network of multidisciplinary professionals, enhancing the care or people with Diabetes at local level, promoting evidence based practice to ensure efficient and effective use of expert resourcesTo participate in the decision-making and policy reviewsTo support the implementation of the Diabetes National Service Framework for the Somerset Primary Care Trust Responsibility for Finance, Equipment & Other ResourcesTake ownership of equipment supplied to enable job role to be fulfilledContribute to prudent stock control/maintenanceThe post holder will assist in monitoring the Diabetes Services budget.
