Summary

Work within our Central Referral Centre (CRC) on site, in Taunton, managing enquiries from Patients and Health Care professionals to our Advice Line, triaging referrals and responding as appropriate. Shift patterns are 8:00 am to 6:00 pm and 2:00 pm to 10:00 pm, supporting a 7 day service. Effectively identifying the needs of patients and those close to them by assessing, planning and implementing care within a holistic model, both remotely and face to face is essential to . Supporting and advising patients, using specialist knowledge and skills in physical symptom control, social, emotional and spiritual care. Working with methods of quality improvement in order to ensure change is safe, well led and responsive. Working collaboratively with the hospice multidisciplinary and external teams to provide care, support and direction. Providing advice, support and education to other health care professionals, where appropriate, as part of a multi professional approach. Supporting all aspects of the clinical referral process, triaging each referral and identifying response and needs based on assessment skills. If a home visit is required, based on need, conducting visits and feeding back to the community CNS team. Assessing the appropriateness and urgency of referrals, seeking further information as appropriate, managing expectations and giving clarity to outcomes. Liaising with other teams within St Margaret's hospice care, following triage and assessment process. Managing advice line calls and acting as the first line of support and advice.
