Responsibilities
Conduct comprehensive assessments of service users' needs and risks.
Develop and implement care plans in collaboration with service users and their families.
Supervise and mentor junior social workers and practitioners within the team.
Provide expert advice and guidance on complex cases to ensure the best outcomes for service users.
Collaborate with external agencies and stakeholders to coordinate care and support services.
Monitor and review the effectiveness of care plans, making adjustments as necessary.
Ensure compliance with all relevant legislation, policies, and procedures in social care.
Requirements
A recognized social work qualification (e.g., Degree in Social Work, DipSW) and valid registration with the relevant regulatory body.
A minimum of 3 years of post-qualification experience in a social care setting, particularly in assessment roles.
Strong knowledge of current legislation, policies, and best practices related to social care and safeguarding.
Demonstrated experience in conducting complex assessments and developing person-centered care plans.
Excellent communication and interpersonal skills, with the ability to work effectively in multidisciplinary teams.
Strong organizational and time management skills, with the ability to prioritize tasks effectively.
Commitment to continuous professional development and staying updated with trends in social care.
Hours Per Week: 37.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £18.15
