Hospice at Home Servicehas been operating for over 2 years with the main focus on caring for patients who want to be at home in the last 4 weeks of life. Many people and their families tell us that they want to stay at home as their illness progresses. For the last part of their lives they want to focus on what matters most to them with those close to them. This is central to how we provide expert care and support.
There will be an induction for any training requirements on starting.
TheHospice at Home serviceis a 24 hour/7 days a week service, based at the Macmillan Unit, Christchurch Hospital, predominantly visiting patients and families in their own homes to provide end of life care. The patients will have complex needs and referred by teams within Specialist Palliative Care Service.
We are seeking applicants who have experience of providing palliative and end of life care, who have confidence to work independently and are passionate about "What matters" for patient's.
The service was started over 2 years ago and has developed significantly during this period. As a Staff Nurse you will complete initial and ongoing assessment of care required. Provide hands on physical end of life care including single administration of medications, Verification of death and last offices. You will need to be flexible with work pattern to include nights.
We are looking for enthusiastic, compassionate, motivational nurses to be involved in providing excellent end of life care. A background in Palliative Care, in providing end of life care or working in the community would be advantageous.

Ifrelocation of the role/service, excess mileage allowance / reimbursement will not apply, and any travel between the postholder’s home and work base will be classified as a commute. Any other changes unrelated to this, will be handled in line with Trust/National terms and conditions.
