Overview
We are seeking a dedicated Senior Care Coordinator to join our team in providing exceptional care services to our elderly clients. The ideal candidate will have a passion for supporting the elderly and ensuring their well-being.

Duties
- Coordinate and oversee care plans for elderly clients
- Conduct assessments to determine individual care needs
- Liaise with healthcare professionals, families, and caregivers to ensure comprehensive care
- Monitor and evaluate the effectiveness of care plans
- Provide emotional support and guidance to clients and their families
- Maintain accurate records of client interactions and progress

Experience
- Previous experience in a care coordination role is essential
- Knowledge of health and social care services for the elderly
- Strong communication and interpersonal skills
- Ability to work well under pressure and handle challenging situations
- Proficiency in Microsoft Office applications
