Summary
Role Overview: As a Senior Practice Nurse, you will play a key role in our clinical team, delivering a wide range of nursing services to our patients. You will be responsible for assessing, planning, and implementing patient care, as well as providing support and guidance to our excellent established nursing team. This is an exciting opportunity for an experienced nurse who is passionate about primary care and is looking to make a difference in the lives of patients. Key Responsibilities: - Provide comprehensive nursing assessments and deliver high-quality care to patients across the lifespan both at the practice and by visiting patients in the community. - Manage chronic disease clinics, including asthma, diabetes, hypertension, and other long-term conditions. - Perform wound care, immunisations, health screenings, and other clinical procedures as required. - Support the development and implementation of practice protocols and guidelines. - Mentor and support junior nursing staff and students, fostering a culture of learning and development, as well as input in recruitment. - Collaborate with the multidisciplinary team to ensure holistic care for patients. - Participate in quality improvement initiatives and contribute to service development. Essential Qualifications & Experience: - Registered Nurse with a valid NMC registration. - Significant experience in a primary care setting, with a strong understanding of chronic disease management. - Advanced knowledge of nursing practices, clinical procedures, and patient care standards. - Excellent communication and interpersonal skills, with the ability to build rapport with patients and colleagues. - Strong organizational skills, with the ability to manage competing priorities. - Evidence of ongoing professional development and training. Desirable Qualifications: - Experience in a senior nursing role. - Additional qualifications in areas such as cervical cytology screening, chronic disease management, spirometry, or travel health.
