Description

Responsibilities
Conduct thorough assessments of clients' social care needs
Develop comprehensive care plans in collaboration with clients and stakeholders
Review and update assessments regularly based on changing needs
Provide information and guidance to clients and their families about available resources
Advocate for clients' rights and ensure they receive appropriate services
Collaborate with healthcare professionals and community organizations
Maintain accurate and confidential records of assessments and interventions
Requirements
Bachelor's degree in Social Work, Psychology, or a related field
Proven experience in social care assessment or a similar role
Strong understanding of social care legislation and best practices
Excellent communication and interpersonal skills
Ability to work effectively in a multidisciplinary team
Sound analytical and problem-solving abilities
Commitment to ongoing professional development and training
