CLINICAL Be accountable for the direct delivery of care to a caseload of patients assisting in carrying out comprehensive assessments in order to plan, implement and evaluate care delivery Demonstrate clinical knowledge of skills in respiratory medicine in relation to the assessment, co-ordination and facilitation of admissions to hospital, instigation of home IVs and management of in and outpatient care. Comprehensive assessment, planning, implementation and evaluation of care within a variety of settings. Working independently using initiative to analyse, problem solve a range of clinical issues whilst working within the parameters of safe practice. Awareness of the social and psychological impact of the disease. Involvement in assessment of these aspects of care, when to refer to outside agents and an ability to communicate and support through advanced communication skills Awareness and implementation of safeguarding measures. Assistance with co-ordination of care for people with CF Diabetes Supporting the transition of new patients from paediatric services over to adult care. Assistance with co-ordination of care for patients referred and accepted for transplantation. Co-ordination of care and support for people with CF towards their end of life Demonstrate an awareness of research and evidence-based practice and clinical excellence and be involved in the introduction of research-based changes to improve patient centred care with the support of the CF team. Demonstrate a comprehensive understanding of accountability, MANAGERIAL/LEADERSHIP Demonstrate professional leadership and assist in the writing of both clinical and developmental policies, protocols and procedures for this service. To liaise with other nursing staff to promote teamwork and to ensure equality of service provision across the Trust for people with Cystic Fibrosis Demonstrate a commitment to quality and service development. Evaluate service provision by participating in audit and implementing action plans and change based on the results. Participate in the day to day running of the Cystic Fibrosis service ensuring that there is sufficient flexibility to meet the needs of patients. Maintain safe standards of practice at all times and report any concerns to a senior manager / team member Be aware of and report promptly any issues that may affect health and safety, security or patient safety and prompt best practice in the clinical area. Keep and maintain accurate records and ensure that confidentiality of information is always assured. Have a knowledge of and work within the policies and procedures of the Trust. ORGANISATIONAL / EDUCATION Be responsible for identifying own training needs and maintaining own professional development. Use specialist knowledge to assist and support with teaching relating to Cystic Fibrosis, to staff and students Initiate and participate in teaching people with Cystic Fibrosis and health care professionals both in the hospital and community setting. Support students and act as a mentor when required. Continually update nursing practice in accordance with current research and evidence in nursing and specialist practice Assist in providing a full range of support, counselling and ongoing education to known patients and carers and in addition for those patients that will be new to the Trust. To gain an awareness of the specialised field of nursing at professional meetings, study days and conferences, including active membership in the CF Nursing association Discuss personal and professional development needs during and annual appraisal with manager and agree and document action points in a personal development plan. PROFESSIONAL Maintain active status on the NMC register. Act as a role model and mentor to all nurses. Be responsible for identifying own training needs and maintaining own professional development. Continually up-date nursing practice in accordance with current research and evidence in nursing and specialist practice Discuss personal and professional development needs during and annual appraisal with manager and agree and document action points in a personal development plan. To further develop the role of the Cystic Fibrosis nurse specialist. Identify own learning/development needs and take steps to pursue objective. Willingness to work independently and use initiative. Manage time effectively, prioritise and organise workload. Identify, analyse and solve problems from a clinical perspective. QUALITY Acts as a patient/staff advocate through the application of ethical, legal and professional knowledge and skills. Engage in effective communication with patients, relatives and visitors, often delivering complex and highly sensitive information. Demonstrates effective verbal and written communication with all members of the multidisciplinary team within own department and relevant departments, which service the clinical area. Makes justifiable referrals to a wide range of other healthcare professionals and specialities, which may include the admission, or discharge of patients from hospital. Provides support, empathy and reassurance in the delivery of patient care. To have skills to communicate learning on an interpersonal level and in presentations Contribute to a supportive environment in the interest of staff morale. Develop external professional networks, which promote both the profession and organisation.
