We are a close and supportive team of Social Workers, Social Care Practitioners, Care Advisors, Senior Practitioner and Service Manager. Our team members have manageable caseloads, and we recognise the importance of robust reflective supervision. You will be encouraged to share your ideas, exhibit best practice, and actively work towards improving the work that we do.

This is a full-time permanent vacancy of 37 hours per week however part time requests are welcome and considered. Flexibility maybe required to support our services.

We work with adults over 18 and above with a variety of presenting needs, which provides opportunity for a stimulating and interesting caseload.

We are a short-term service providing short -term support to service users in the community, working closely with the reablement service, Care Coordination Centre Triage Clinicians and Community Services.

As a Social Worker in the First Response Team, you will be:

Undertaking assessments under the Care Act 2014 and the Mental Capacity Act 2005. This involves direct work with individuals, families, and professionals.
You will be responsible for Care Planning and providing short term case management to adults in need of care and support.
Attending multi-disciplinary meetings and completing initial Safeguarding enquiries.

You will be based primarily in Peasedown St John, Bath but will work across the whole of Bath and North East Somerset region.

About You

You must be a qualified social worker and be registered with Social Work England.

Ideally you will have experience of working in an Adult Social Care duty team. however, if you do not have this experience yet, please still apply as we are committed to providing training and development to further your potential and support your progression.
