Summary
Please see Job Description and person specification for this job The post holder will be required to work with partners across the health and social care to ensure a joined up and integrated approach is adopted towards the discharge process and provide holistic care. The post holder will support and oversee the team management, co-ordination and provision of service. The post holder will involve in supervision and development of junior staffs. The post holder will be an active participant in shift work, covering either 8am-4pm, 10am-6pm, 9am-5pm or 12-8pm with rotad weekends and Bank holidays as the service operates 365 days per year.
