Summary
We are looking for a Paediatric Diabetes Specialist Nurse to come and work within our established multi-disciplinary team. This is an exciting opportunity to join an award winning team who have vast experience in the recent advancements in diabetes care for children and young people with type 1 diabetes, including closed loop pumps and CGM.
The team cover both Calderdale and Huddersfield, however the post will initially be in the Huddersfield area. The team are based at the new Rainbow Community Hub in Elland.
This role involves visiting patients in their own homes and visiting schools on a regular basis and therefore a car is essential for the role
To provide evidence based specialist care to children, young people and their families with Type 1 Type 2 diabetes in Calderdale and Huddersfield
To manage your own caseload of patients and to make clinical decisions on a daily basis, involving altering insulin doses, offering telephone support to children, young people and their families, as well as supporting school staff and other professionals
To attend multidisciplinary clinics on a weekly basis,
To attend MDT meetings within the team as well as regional Nursing Network meetings
To attend multidisciplinary meetings supporting children that require safeguarding intervention
To commence pump starts and provide continued support and information to the children on pump therapy
To provide structured education to families and relevant professionals involved in the care of the children and young people
To be involved in compiling audit data

You will be responsible for managing your own caseload alongside the dietitian and patient advocate. You will also be responsible for supporting patients that are in our transition service alongside our adult colleagues and attending the evening transitional clinics.
You will attend regular follow up clinics, new patient clinics and nurse led clinics as well as visiting patients at diagnosis on the ward, and continuing their education in the community setting
