Summary


The BLOSM Navigator will work integrally as a member of BLOSM service in part of a multi-disciplinary team. The purpose of this role is to support vulnerable people in Emergency Department (ED) with needs such as, but not exclusive to, substance use, social isolation, life skills, housing, criminality, assault, exploitation and relationships.

As a Navigator, you will be working in ED and community to support children and young people (11 – 25) with some of the issues mentioned. Using trauma-informed practise, you will need to engage in various forms of communication to support them at a difficult time. You will assist these people and build trusted relationships, getting to know their needs and which service they will be best placed. It is essential you can communicate with others at all levels, and liaise with colleagues in other hospital departments, statutory and third sector services, to ensure excellent coordination and support for the person you are working with.



The post holder will be employed and accountable to the organisation.
• Provide trauma informed practice to people in ED and signpost to members of the team for follow-up in the community in some cases.
• Provide non-clinical advice to young people, families, and carers on a wide range of subjects.
• Maintain communication with the wider MDT when appropriate, these disciplines include Health and Social Care, Police, Education and Voluntary Sectors.
• Attend relevant training in relation to these complex issues to ensure that the competency needs of healthcare staff and others are addressed.
• Advocate for the person to support them in working towards their personal goals.
• Liaise with other professionals to promote health and social wellbeing for individuals.
• Represent the BLOSM team and promote the work for the BLOSM service within the trust.
• Support people and carers to access appropriate information and support, including sign posting to a range of support services.
• Experience of discussing ideas and assessments in a professional way, which supports the person in their best interests, communicating their thoughts and wishes.
• Ability to work on own initiative within clearly defined policies and procedures, seeking advice as necessary from the Operations Manager.
• Ability to focus and concentrate frequently when providing advice.
• Maintain accurate electronic patient records and other data resources.
• Flexibility to work within areas or departments to meet the demands of changing service needs.
• To support other colleagues when required in line with your competencies.
• The postholder will frequently work with individuals who are experiencing distressing or emotional circumstances.

We provide community health services, including sexual health services in Calderdale from Calderdale Royal and local health centres. These include Todmorden Health Centre and Broad Street Plaza.
