The East Midlands Planned Care Centre (EMPCC) is a new Hub for elective care that envisaged to be fully operational in December 2024. The addition of the EMPCC will enhance services at the UHL and enable the Trust become a leading organisation in healthcare delivery and reshape the way we work. This is a critical step towards reducing waiting times, improve patient and staff experience.
The EMPCC aims to provide a medical model that has a focus on minimally invasive and ambulatory care that has enhanced early recovery at its core. .
The Gynaecology Team is delighted to one of the key services that will have a base from the EMPCC. This will also enable significant expansion in workforce and a unique opportunity to restructure our model of care. The new post holders are envisaged to work in an integrated manner with existing colleagues . The job plans are designed to enable the development of special areas of interest and expertise.
Recruiting to the advertised clinical roles is a major step toward enhancing the services in region and we aim to appoint highly motivated staff who have the necessary skill and ambition.

Key Tasks
· To run ambulatory, inpatient theatre and/or day case and lists.
· To run outpatient clinics.
· Take part in governance and case discussions and care planning.
· Contribute to the review of post-operative patients following surgery.
· To contribute to the day-to-day running of the Emergency Gynaecology Service in the Gynaecology Assessment Unit at the LRI and on call rota as agreed by Head of Service for Gynaecology. This includes emergency gynaecology on-call, twilight cover and hot weeks and to provide support to the Obstetric Service in emergencies.
· To contribute to e-Referral Service Advice and Guidance (online advice to GPs)
· Maintenance of the highest clinical standards in the management of patients.
· To share with colleagues responsibility for the day-to-day management of patients.
· To promote new ways of working and co-ordinating care for patients in the community and to ensure that services are based on effective and integrated partnerships across the health community.
· Supervise and train junior medical staff.
· To be involved in appraising and assessing juniors.
· Teaching, research and administration.
· To proactively develop the service.
To actively participate in both departmental and Trust matters concerning Clinical Governance and audit.

Job Plan
The job plan will be subject to annual review, with any revisions sought by mutual agreement but it is expected that the successful candidate will work with the CMG management team to ensure that services are delivered efficiently and reflect best practice with processes and governance to match - this may require new and flexible ways of working in the future.
· The clinical sessions are currently 8:30-12:30 & 13:00-17:00 i.e. 4 hour sessions. Any variation needs to be agreed by the Head of service and the general manager.
· Theatre sessions include additional 30 min for pre and for 30min for post op ward rounds (1.25PA).The job plan may contain additional time for patient related admin under the titles A&G and ERS. It also contains protected time for MDT attendance.
· For clarity the Trust currently operates from the (LRI) Leicester Royal Infirmary Site and the (LGH), Leicester General H Site. The trust is in the process of reconfiguration to a Women’s Hospital on LRI site and EMPCC on the LGH site.
· The post-holder will participate in hot week emergency gynaecology cover and Twilight shifts at the gynaecology assessment unit which is currently located at the LRI site. These shifts cover gynaecology. The post-holder will work collaboratively with the service and colleagues should there be a need to develop a different service model for emergency cover.
· On-call consultant is expected to respond to emergency gynaecology patient attendance to the gynaecology admission unit (GAU). Emergency (GAU) is currently at the LRI site. The consultant is expected to support obstetrics in the infrequent instances where gynaecological input is required. On call duties are non-resident from home between 2030h and 0800h unless there are unwell patients needing consultant attendance and are currently in band A. Theconsultants will need to live within 20 mins from the hospital or seek suitable overnight accommodation closer to the LRI. Daytime hot week sessions start at 0800h to 1700h, with Twilight cover from 1700h to 2030h.
· (Unless otherwise agreed) There is no Labour ward or obstetrics cover in the Job plan apart from the expectation of being able to provide gynaecological input if called upon by Consultant Obstetricians covering labour ward or elective obstetrics theatre.
The post-holder should maintain the necessary skill to enable safe management of gynaecology emergencies at a tertiary level provider accepting complex acute gynaecology patients from the region. The UHL does not refer emergency gynaecology patients elsewhere as it is a large teaching hospital and has the necessary multidisciplinary input to provide support to neighbouring units if required
· The on-call rota will not exceed 1:10 coveringGynaecology onlyat LRI (emergency admissions - includes a resident twilight session for Acute Gynaecology) and occasionally supporting obstetric emergencies (eg for hysterectomy for obstetric emergencies). Emergency work has been recently assessed as averaging out to 1 programmed activity per week. Mentorship will be provided where necessary.
· Shared office space based on agile working basis and secretarial support is provided.Each Consultant is provided with a personal laptop computer with access to the internet to allow working for home for non-patient facing sessionsand NHS.net connection.
Outline Job Plans
The posts will involve working 3 Saturdays in 12 (1 all day general Gynae clinic and 2 all day Day case lists) but no weekend on call work.
When a consultant has worked the Saturday, they will have a compensatory day off on the subsequent Monday.
Mondays will be worked on a 12 week rolling rota to complement the Saturday work entailing daycase theatres, general Gynae clinics or SPA
Tuesday- Fridays will be set days of work as per the job plan and will include 1 further OFF sessions
Access to main theatre operating will be flexibly allocated on either a morning or afternoon session of the set theatre day, booked 6 weeks in advance with the consultants own patients or pooled patients listed depending on prioritisation of the patients.
Other DCC sessions will be scheduled as per skills of the successful applicants and are expected to include: diagnostic and ambulatory hysteroscopy (morcellation and ablation) or colposcopy.
Hot weeks will be split M/T/W and Th/F – and 2.5 whole weeks of hot weeks will be rostered per year in rotation with all the general gynaecologists
On call cover at LRI (emergency site) will be provided on M/T/W/Th/F on a 1 in 20 rota in addition to an additional Twilight session 1 in 20.
