Summary
Attend and actively participate in Multi-Disciplinary Meetings and daily huddles. Contribute to the provision of unplanned activity through allocated same day appointment slots. Access/conduct peer/individual supervision and receive supervision from agreed supervisor on a regular, planned basis, and work within the appraisal process (PDR). Contribute to the development of others through supporting students, contributing to informal supervision, seeking advice and support from others to enhance the offer of students experience. To be committed to individual professional development, ensure compliance with mandatory training and support development of the STR role. To maintain accurate and up to date records of all contacts and interventions with service users. To undertake any other duties that maybe allocated by the Team Manager within the general scope of the post and appropriate to the grade. It is the responsibility of all Trust employees to ensure that all duties are carried out in accordance with best practice risk management. Work within the framework of relevant policy and legislation both local and national. Assist in the design and implementation of service development Providing a screening service for clients under the care of the Community Mental Health team as outlined by the named worker above, ensuring the service user can access the correct information/intervention in a timely manner. To empower and involve service users in the design of their own care and in the delivery of this and the wider service provision. To offer a flexible service, which supports service users in times of distress or mental health relapse, involving relevant others where necessary, and increasing support on a reviewable basis. To identify and effectively manage and escalate risk issues and taking the appropriate action in times of crisis and emergency, to ensure the safety of the service user and all concerned. Support and provide direction to other recovery workers. To undertake risk assessment for any work undertaken where appropriate, with particular reference to lone working policies and environmental risk. Develop a rapport with service users within appropriate boundaries. To encourage service users to be fully involved and engaged with their agreed care plan and to access appropriate services. Implement agreed programmes of care collaboratively with service users and carers under the supervision of a named professional. Help to identify early signs of relapse by monitoring the service user's progress, level of functioning and mental state and alert the appropriate staff involved in their care. To take a role in evaluation of care provided To identify and support carer needs as required. To take a role in health promotion including physical health needs Participate in and help run social activity groups.
