12 month Fixed Term Contract
We are looking for somebody to provide a wide range of high quality Community Mental Health Care Services which will enable people to live as independently as possible and have meaningful choices.
As a member of the Primary Care Network Mental Health Team, the post holder will provide high quality Nursing Interventions, Medicine Management, Severe Mental Illness (SMI) Health Checks and support Wider System Pathways, as well as other Specialist Mental Health Services. The post holder will assist in the development of this new initiative, working collaboratively with other agencies to ensure patients receive high quality and standards of care appropriate to their identified mental health needs.
The post will be linked to one specific Primary Care Network (PCN), however the post holder will work across the four Thurrock PCNs to support colleagues and maintain service continuity as appropriate.
As a Primary Care Mental Health Practitioner you will be a highly skilled and motivated Mental Health Nurse, or Allied Healthcare Professional, and be part of the Enhanced Primary Care team at the forefront of establishing this innovative development in Primary Care Mental Health Services. As a highly competent and proficient practitioner you will be able to work autonomously and be confident in your ability to provide evidence based psychological and non-psychological treatments for common mental health disorders.
This will be in the context of a wider programme of mental health service transformation aimed at creating more capacity and supporting services to become more inclusive and accessible by removing boundaries between services and organising care, based on individual need. The role will actively change the way that services are designed and delivered for people experiencing severe and multiple disadvantage in Thurrock.
