A great opportunity to join the Suffolk County Council's (SCC) Mental Health East Team as a Senior Independence and Wellbeing Practitioner, based at Landmark House, Ipswich and covering Mid and East Suffolk.
Are you passionate about supporting people to be as independent and as well as they can be and enjoy identifying personalised and creative ways to meet the needs of adults requiring mental health support? If so, this role could be for you, and it could also enable you to progress towards a social work qualification.
Your role and responsibilities
You will:
- have a passion for improving the lives of adults with mental health needs and for developing your own skills and those of your colleagues. We are looking for someone who thrives on working in a strengths based, enabling and personalised way
- work in a variety of community settings including family homes, residential and supported living services, day services and community facilities
- be responsible for working with customers, their families, health colleagues and service providers to carry out asset-based customer reviews, as well as supporting those caring for our customers
- work to our SCC strategy and Signs of Safety and Wellbeing practice model, promoting independence and self-support and personalisation, to identify with the people we support and their carers creative ways in which their needs may be met
- undertake comprehensive service reviews that identify what is working well and how services can improve.
We have an advanced digital care offer and will be keen that you explore this support alongside other technology.
You will need
- effective communication skills
- demonstrative awareness and understanding of people’s situations
- good knowledge of the assessment, support planning and care management process, including of the relevant legislation
- demonstrative ability to keep timely, full and accurate records.
