Summary


An exciting opportunity has arisen for a IRIS Co Ordinator to join the Team at Frimley Park Hospital.

The post holder requires excellent organisational and communication skills, working in partnership with professional staff in other agencies and service providers.

Needs to be highly proficient in IT skills, including, Word, Excel and has experience of working with EPIC and to have the ability to work independently, flexibly, be adaptable to change and also work as part of the wider multidisciplinary team.

Day to Day Co Ordination of the IRIS office at Frimley Park Hospital

Partnership working with health and social care partners both within the acute hospital and community settings, particularly in relation to patient discharges

Triaging referrals

Administrative duties, including meetings, email and diary management

Data Entry

Work as part of a rota covering 8am -6pm 7 days a week

To facilitate the most appropriate discharge pathway for patients leaving hospital

To support cross organisation working and information sharing

To act as a single point of access to community services for patients who have been admitted and/or are needing support on discharge

To share information and signpost people to the most appropriate service

To co-ordinate the day to day running of IRIS at Frimley Park Hospital

To facilitate appropriate information sharing in relation to supporting patient discharges

To triage referrals received in IRIS, forwarding them to the correct service for the patient’s discharge

To use the EPIC system to track patients and pull reports
