Summary
Assess, diagnose, triage, and manage patients, taking responsibility for the management of a caseload. Develop and make use of your full scope of clinical practice, including skills relating to supplementary prescribing and investigation when trained to do so. Support patients to set their own goals and be confident in their approach to self-manage their conditions where appropriate e.g. diabetes, high cholesterol & cardiovascular disease. Provide care which is tailored to the individual needs. Make prevention possible in primary care with dietetic interventions for conditions including but exclusive to: type 2 diabetes, lipid lowering and hypertension. Work with the PCN to provide an equitable service which minimises health inequality Liaises with PCN colleagues including commissioners, to ensure consistency of patient care and benefit to GP practices. Supervise junior members of staff by taking part in 121 and appraisals Supporting student training
