Job Summary: We are seeking a dedicated and compassionate Case Manager to join our dynamic team. The ideal candidate will be responsible for managing their clients, developing personalised care plans, and coordinating necessary services to meet their unique needs. This role requires excellent communication, problem-solving skills, and a strong commitment to client advocacy.


Key Responsibilities:

Conduct comprehensive assessments of clients' needs, strengths, and preferences.

Develop and implement individualised care plans in collaboration with clients, families, and healthcare providers.

Coordinate and monitor the delivery of services, including medical care, social services, housing, and financial assistance.

Advocate for clients' rights and needs, ensuring they receive appropriate and timely services.

Maintain accurate and up-to-date client records, documentation, and reports.

Provide ongoing support, counselling, and crisis intervention as needed.

Facilitate communication between clients, families, healthcare providers, and community resources.

Participate in multidisciplinary team meetings and case conferences to ensure holistic care.

Stay informed about changes in regulations, policies, and best practices in case management and social services.


Qualifications:

Health Care professionals with a minimum of 2 years of experience in case management.

Strong knowledge of community resources, healthcare systems, and social service programmes.

To be a registered member with BABICM or CMSUK

Excellent interpersonal, communication, and organisational skills.

Ability to work independently and as part of a multidisciplinary team.

Proficiency in electronic health records (EHR) and case management software.

Valid driving licence and reliable transportation for client visits.
