Summary

Cheshire East Council (CEC) is looking for experienced and motivated Grade 9 Social Workers to work in Short-Term Services, which support hospital discharge to assess services in the East of the Borough. Applicants will need to be able to undertake person-centred and strengths-based assessment and support planning for people in Acute Hospitals. Assessments are also carried out in residents own homes or short-term care settings.

The Short-Term Services team works to Home First principles, and in close partnership with colleagues in acute and community health services, social care, and the voluntary sector. The local health and social care economy are working to embed and improve Home First working and with continued service development it is an exciting time to join the team. The team is community based with in-reach into hospital and community bed settings. Home working is currently available, and the Council policy is a model of long-term hybrid working. The team is innovative, friendly, and supportive, and there are good opportunities for training and professional development within the job role.

This is an exciting opportunity for an experienced Social Worker to undertake a role which requires the use of core skills, knowledge and Social Work values including good communication, decision making skills and the ability to work under pressure. The successful applicant will need to be able to work autonomously and at pace, have a flexible approach to their work and be a good team participant.
We are looking for:

Enthusiastic and experienced Social Workers who have ideally have experience and knowledge within a hospital setting, knowledge of hospital processes and pathways in this fast-paced environment. Experience of adult safeguarding, the Mental Capacity, Care Acts, and best interest decision.

Candidates who are proactive, with excellent communication skills and the ability to work autonomously and in partnership with health colleagues. They should have good organisational and communication skills.

Social Workers who can demonstrate evidence-based practice, sound knowledge, and be able to manage care needs in a variety of settings.

Candidates who work within Social Work professional practice requirements, Council policies and who will proactively pursue professional development opportunities.

The post is based in the East of the Borough, the team are based in the Macclesfield area but support individuals across the whole of the borough (predominately Macclesfield, Poynton, Knutsford, Congleton and Holmes Chapel). It is a 37 hours per week for 18 months.
