Please see the attached document for the full job description. QHS GP Care Home Service provides primary medical care for residents of all older adults care homes across Southwark. There are seven care homes which all receive dedicated weekly visits for GP ward rounds ensuring continuity of care. The QHS GP Care Home Service team also includes a dedicated clinical pharmacist, nurse extensivist, nurse associates, dementia specialist nurses and a care coordinator. There are regular MDT meetings with the care homes, local geriatricians, social workers, mental health team and dieticians that enable us to deliver truly holistic, long-term, multidisciplinary care not seen in most primary care settings. QHS, has been appointed by North Southwark PCN to deliver Enhanced Access Service to the North Southwark population. This service was initially designed to provide additional capacity for member practices catering acute presentations, however over the recent years, we expanded to incorporate selective routine primary care services to support local needs. New Mill Street Surgery is a small, but dynamic and forward thinking GP practice of 7,000 patients in the heart of Shad Thames. As it is run by Quay Health Solutions, New Mill Street Surgery is at the forefront of testing new ways of working in the borough and hosts a variety of clinicians (including GP's, ANP's, Nursing Associates, FCP Physios, Clinical Pharmacists and Mental Health Practitioners) resulting in a well-rounded and diverse clinical team. QHS is also working with NHS Southwark ICS on a programme of development to support transformation of local health services. We are working collaboratively with providers and patients to co-produce solutions to the challenges we face and maximise the impact of our services on quality and outcomes of patient care.
