Summary


This is an exciting permanent opportunity for a part time Occupational Therapist working 22.5 hours a week.The ESD for stroke team, based at St Marks’Hospital isa supportive multi-disciplinary team (MDT)comprisingNeuro Specialists in Psychology, Physiotherapy, Speech & Language Therapy, Therapy Assistants& Stroke Nurseswho deliver high quality assessments and rehabilitation for patients who have been discharged from hospital following a stroke, related to their individual goals and needs.


You will receive supervision and support for your caseload from experienced Occupational Therapists in the team and from the wider Community Neurological rehabilitation service.

Your professional development will be supported and encouraged.Clinical care will be provided in patients' own homes, orotherappropriate communitysettingsdependent on patient goalswithin East Berkshire,but some cover to the West of Berkshire may also berequiredas part of the wider Community Based Neuro-rehabilitation team. The successful candidate must therefore be a car driver and have access to a car. We are a well-respected,establishedand forward-thinking team where there is a culture of continual improvement to achieve exceptional outcomes for patients.


The role will provide specialist Occupational Therapy to adults with Neurological conditions that require Rehab as part of the community neurorehabilitation and Earlier Supported discharge teamfor Stroke. You will complete specialist assessments, plantreatmentand provide rehabilitation to patients who may have stroke impairments such as physical,visualand cognitive changesimpactingon theirfunctional performance.


You will work jointly and effectively communicate with MDT colleagues to achieve positive outcomes for your patients. You will use your skills to empower patients and their families to increase their independencethrough inspiring self-management and engagement in rehabilitation.


You will be able to manage your time and prioritise your demands to ensure the needs of each patient are met appropriately with the support of the MDT. You will have opportunity toparticipatein team training and service development. There is opportunity to also develop skills in treating progressive neurological conditions as part of the wider Community Based Neuro Rehab Team.


The post will be offered to individuals who are motivated, innovative and have experience ora strong desiretodevelopclinically as part of adynamic, client centred service.
