As Community Care Assistant, you will contribute to assessment, risk assessment, personal outcome and recovery focussed care planning and review, and will be directly involved in the delivery of support to adults with mild to moderate mental health needs living in rural towns and villages.

As Community Care Assistant, you will plan, organise and manage the delivery of support in line with policy, procedure and best practice. The support offered will be short term and recovery focussed, and will encourage the development of self-management skills to support service users to move on from, or live better with, the symptoms of mental ill health. Support can be delivered on an individual or small group basis.

The Community Care Assistant role within the Wellbeing Support Team requires qualification to SVQ3/HNC in Health and Social Care level, and some previous experience in mental health support work.

This post will be based in Aberfeldy but the post holder may be asked to work across other areas of rural Perthshire in line with service needs.

You must have a valid UK driving licence as this post requires you to travel to a variety of locations across Perth and Kinross.

The Individual

The successful post holder will demonstrate good written, IT and verbal communication skills, and demonstrate skills in assessment, care planning and review processes.

You must have a genuine interest in working with people in a recovery focussed way to improve their mental health and wellbeing. You will be able to demonstrate skills in group-work facilitation.
