Summary


The post holder will be joining a well-integrated and established multi-disciplinary team providing nursing care to a varied caseload of patients aged from 18 years old and upwards. And working closely with our Integrated Care services to provide holistic care.

To provide and monitor safe, person centred care to patients in a range of care settings under the direction of a Registered Nurse, without direct supervision in line with an agreed plan of care.

To support the registered nurse in the contribution to on-going assessment, treatment and care planning for patients

To adhere to the Nursing and Midwifery Code of Conduct for Nurses, Midwives and Nursing Associates and work within their scope of practice following the professional standards of practice and behaviours for nurses, midwives and nursing associates and the standards of proficiency.

To recognise changes in a patients' condition which require the intervention of others and understand when to refer to the supervising registered nurse and other colleagues for re-assessment of patient care.

To be involved in Trust initiatives to improve patient safety and care including participating in quality assurance visits and audits.

Report any accidents or incidents and raise any concerns as per organisational policy and demonstrate an awareness of risk management and clinical governance.

Identify risks to safety or experience and take appropriate action, putting the best interests, needs and preferences of people first.

Reporting/ escalating any safety, safeguarding or risk concerns to the appropriate member of staff and ensuring that action is taken post reporting.
