Summary

Personal Profile: Candidates must meet essential criteria and be willing to undertake an advanced practice pathway at level 7 master's degree including non-medical prescribing. Be flexible and prepared to work across a 24/7 service covering all East Lancashire HospitalTrust (ELHT) sites. Exercise advanced clinical expertise, level of judgement, discretion and decision making inclinical care of patients, which may include highly complex medical and surgical presentations. Must have experience within an acute setting and have worked at a senior level for a minimumof 3 years. Have recognised skills to function at a high level to support the Acute Care Team rota. Be prepared to undertake rotation throughout clinical areas to develop required and recognisedskill set that support deteriorating patients, including critical care, emergency medicine, andcommunity and integrated care settings. Role: To provide expert advice, support and direction to ensure holistic patient care for acutely unwell and deteriorating patients with the multidisciplinary team. Work closely with the critical care nursing and medical teams to facilitate early and timelyadmissions, safe discharges, and follow up and rehabilitation care. Collaborate with the deteriorating patients steering group and division of Education, Researchand Innovation to develop and deliver acute illness management education. Provide the infrastructure that addresses the knowledge and skills deficits of clinical areas indelivering acute care skills to patients. Support the Implementation of national guidelines and / or evidence-based practice to improvepatient care. Addresses clinical governance issues promptly. Provides direct leadership for the Acute Care Team. Provide highly specialised care within the hospital setting, to those patients with chronic and acuteconditions using advanced clinical knowledge that is evidence based Function independently or within a clinical team demonstrating competence complementary to theMDT in the delivery of health care. Provide expert advice and ensure the needs of the patient are met by leading, challenging andchanging practice within the clinical setting. Accept new and follow-up referrals Discharge patients from workload as appropriate Undertake clinical audit and research Recognises and acts as an advocate for patients, carers, service and organisation. Lead change as required and ensure resolution of conflict. Undertake and lead quality improvement projects to support service development. General Responsibilities: Working as a Trainee Advanced Clinical Practitioner making independent clinical judgements/decisions based on patients' clinical need and within scope of competency Undertake independent comprehensive assessment and management of patients to incorporatephysiological, psychological and pharmacological considerations, making multifaceted complexclinical decisions regarding patient management and clinical outcomes, including, whereappropriate, listing patients for elective procedures and discharging patients from the service whentreatment is complete/optimum outcome is achieved Demonstrate a high level of competence in clinical, diagnostic and examination skills Provide counselling and advocacy for patients with acute and chronic conditions Act as a facilitator, resource and model to nursing and medical staff, and professions allied tomedicine Undertake research and service development Regularly re-evaluate the service and develop according to need, within the limitation of resource Establish close liaison and communication between all disciplines involved in the care of patientsand their families and work in partnership with other practitioners Monitor and evaluate the role of the Advanced Clinical Practitioner, liaise with appropriate managersand medical staff, and demonstrate and respond to an evolving service Develop and sustain close working relationships, partnerships and networks and draw upon multiagency/inter-professional resources where appropriate to improve health. Keep up to date with developments in the role of the Advanced Clinical Practitioner through personalprofessional development, in service training and education, professional conferences, courses andseminars as appropriate.
