Summary


We are looking for a Full-Time and Part-Time enthusiastic, skilled and motivated Community Support Workers to join our Quickstart service across Wandsworth and Merton boroughs to deliver high quality, timely care support with an enablement focus to patients in the community.Quickstart service provides rapid response social care services for patients who are at risk of social care breakdown. Without a social care package in place these patients would otherwise be admitted to hospital due to an overriding social care need.



The role is varied and diverse, and will involve following a personalized care plan, providing support including personal care and domestic tasks, across a wide range of client groups including people with dementia, frailty and complex long term conditions.

Quickstart works closely with Maximising Independence Team, a dynamic, multi-disciplinary team of physiotherapist, occupational therapist, speech and language therapist and rehab assistants, allowing for joint working with rehabilitation and learning opportunities. The post will provide an opportunity for the successful candidate to develop their clinical skills and knowledge base.



Fulltime post for 37.5 hours per week (5 days per week) -hours are worked across 7 days/week including bank holidays and weekends, and across shifts covering 07.30 – 22.00, depending on needs of the service. Staff are expected to cover a range of shifts across the weekend, bank holidays and operational hours of 07:30 – 22:00.



Part time post for 22.5 hours per week (3 days per week) -hours are worked across 7 days/week including bank holidays and weekends, and across shifts covering 07.30 – 22.00, depending on needs of the service. Staff are expected to cover a range of shifts across the weekend, bank holidays and operational hours of 07:30 – 22:00.
