Summary

JOB SUMMARY
Be a member of the Early Intervention service being responsible for the delivery of services to people who are residents of Newham with a recent onset of psychosis as defined by the service referral criteria.
Establish a network of relationships with educational, vocational and leisure facilities, and maintain a database/resource of such organisations.
Supervise and co-ordinate the work of trainees/students of your related profession, as appropriate.
Be involved in training/ health promotion of early onset psychosis as required. Interface effectively with community organisations.




Key Responsibilities
• Contribute to the delivery of service to people with recent onset of psychosis.

• Provide specialist assessment and psychosocial/psychotherapeutic skills, together with the provision of wide-ranging practical support.

• Provide assessments within health (CPA) and social care (care management) frameworks.

• Participate in the setting up and running of service user and carer groups, which promote confidence building, meaningful occupation and recovery.

• Provide support to service users in line with a ‘whole team approach’, as recommended in Early Intervention in Psychosis best practice literature.

• Provide a service, which promotes recovery, identifies people’s needs and strengths, is user centred and holistic, and promotes safety and positive risk taking.

• Ensure the service provided is of the best quality and of a standard of professional practice and performance to meet the needs of Newham’s diverse community.

• Provide a service, which adheres to policies and procedures.

• Provide a service, which is of the highest standard in terms of fair access, equality of opportunity and anti-discriminatory practice.

Main Duties and Responsibilities

Direct Clinical

• Management of a personal caseload as agreed with EIS Team Manager and within the set referral criteria.

• Participate in the setting up and running of service user and carer groups, which promote confidence building, meaningful occupation and recovery.

• Undertake care management/CPA responsibilities and provide clinical /physical health assessments and interventions in line with an Early Intervention in Psychosis approach to interaction and management. This includes using an assertive outreach approach when required, as well as implementing recognised specialist psychosocial assessment and intervention strategies.

• Act as a care co-ordinator with an agreed member of service user in line with CPA Policies.



• Establish joint and/or co-working practice for service users where necessary with both adolescent and adult CMHT team members, in-patient unit staff and professionals from other agencies as appropriate.



• Work with other members of the multi-disciplinary CMHT teams and all other relevant health professionals to assess, develop, implement and evaluate high quality packages or service user centred care, as required.

• Visit service users at their choice of venue as far as possible, such as their home, day centres, work and recreational centres, and at venues in the community to engage and encourage service user’s participation in interventions and in all aspects of daily living.



• Undertake (where appropriate) assessment of daily living skills, to ascertain functional ability, and level of support and/or accommodation needs to contribute to community care plans.

• Participate in interventions jointly with relevant voluntary organisations.

• Provide information to help service users make an informed choice about opportunities and services available to them and to then assist service users to obtain the required services, support and entitlement. This may involve acting as advocates for service users and their families/carers.



• Involve families and carers in intervention plans where appropriate, considering issues of confidentiality. This will normally require direct family interventions, to encourage their active support and collaboration with the process of recovery.



• Keep service users and carers (in line with confidentiality protocol) fully informed of their care plan; legal status; intervention etc.



• Ensure principles of risk management and risk assessment are integrated into care plans.



• Administer and/or monitor medication as prescribed in keeping with policies and NMC requirements as appropriate to training.

Indirect Clinical



• Provide support and advice in relation to issues concerning the target service user group or in relation to the EIS worker’s own clinical area of expertise to other team members, relevant health professionals and other agencies as appropriate.

• Liaise with professionals of other services to co-ordinate various aspects of the service, and act as care co-ordinator when appropriate for service users on Care Programme Approach, Section 117 or Supervised Discharge.

• If from nursing profession, to be responsible for the storage and administration of drugs in line with nursing and CMHT policies.

• Carry out statutory responsibilities under the Mental Health Legislation including AMHP responsibilities for those approved by the Local Authority to carry out these duties.

• Be available to other team members for informal support and guidance as appropriate.

• Develop team links and ensure effective working practice with all related Health and Social Services, voluntary, education and employment agencies.
