Summary


Leading a co ordinated and efficient service to patients.

Taking an active role in the education, training and development of Health Care Professionals to promote the objectives of the service.

Promoting early intervention with the most appropriate form of treatment/support which will lead to more effective management of the disease process and an improvement in the patient's quality of life.

Provide clinical advice and support with regard to the needs of patients, to all health care professionals.

Liverpool University Hospitals NHS Foundation Trust was created on 1 October 2019 following the merger of two adult acute Trusts, Aintree University Hospital NHS Foundation Trust and the Royal Liverpool and Broadgreen University Hospitals NHS Trust.

Personally:

The Lead ACP is an autonomous practitioner responsible for providing direct clinical care to inpatients across the site including reviewing patients, ordering investigations, creating management plans, and prescribing. As an ACP, they are also responsible for facilitating learning for colleagues, demonstrating clinical leadership in their work area, and participating in audit and research activities. They will be a source of supervision and support for all grades of nurses and junior doctors.



Organisationally:

The Lead ACP is responsible for management and delivery of the entire ACP workforce on the site ensuring the effective delivery of operational plans and trust priorities. They will work with other stakeholders to ensure delivery of regional and national strategies for advanced practice. The Lead ACP is responsible for developing advanced practice through education, clinical supervision and collaboration with other partners. The Lead ACP will provide assurance and subject expertise in relation to deterioration and resuscitation. They will also act as a clinical lead for the Medical Emergency Team and the Out of Hours Nurse Practitioner team.
