Summary


The Dysphagia Team for Adults with Learning Disabilities (DTALD) provides a service to adults who experience eating, drinking and swallowing difficulties in the community. Dysphagia is a term which means an impairment in swallowing, which may occur with food, drink or saliva. The DTALD team help address any potential health problems caused by being unable to eat, drink or swallow.



The post holder will work as an integral part of a small multi-disciplinary team working in dysphagia and be a key link between Mainstream Community Services; Borough based Learning Disability services and colleagues in local hospitals. In the team there is a specialist Speech and Language Therapist (SLT) and a Specialist Dietitian. This role isto assist the dietitian and speech and SLT in their work with patients, carers and health care professionals to ensure the smooth delivery of the service.

Under the supervision of the dietitian and SLT, the post holder will provide holistic and person centred care to all patients who access the service. They will see learning disability patients with dysphagia, overweight/obesity, malnutrition, nutritional deficiencies, communication needs, patients on enteral feeding and gastrointestinal disorders. Following an initial assessment by the dietitian/SLT, the post holder will review some of these patients in their own home, in care/residential or supported living home or in a clinical setting. Training to clients, parents/ carers and support staff is an integral part of the role. They will also produce accessible care plans and documentation.

Sutton Health and Care (SHC) is an innovative joint venture launched in April 2018 between local partner health and social care organisations, in which Sutton Community Services will be fully integrated to support the deliver a new care model for the people of Sutton. Hosted by Epsom and St Helier University NHS Trust the model ensures the breaking down of traditional organisational barriers to enable the provision of care that is wrapped around the patient. An Alliance Board representing all partners has been formulated to support consensus decision making. SHC vision is to deliver seamless excellent coordinated care that is person centred and of high quality care for people at risk of an unrequired hospital admission, and for those that required admission ensure they are supported promptly to get them home again as quickly as possible after hospital admission.
