Summary


We are looking for a highly motivated person who would like to join our exciting team as a Patient Pathway Coordinators for the General Surgery POD here at Newham University Hospital. This is a substantive role which is full time (37.5 hours). Agile working arrangements can be provided to fit around other members of the team.You will be working within a department, but allocated to a specific specialty. The successful candidate will be scheduling Outpatient appointments for that service, as well as monitoring that patient’s journey within that pathway.

We are looking for individuals with CRS Cerner Millennium knowledge and have job experiences of working in a busy, fast paced division. Experience of working in administrative offices is not essential but preferable.

All staff will receive individualised Induction and Development plans to underpin service delivery in line with the core values of the trust.
To be the point of contact for patients for all issues relating to their pathway of care, both for inpatients and outpatients. The post holder will work as part of a multi- disciplinary team (POD), under the direction of the Clinical Lead, and will provide expertise in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly. The post holder will ensure high levels of patient and clinician satisfaction by being an accessible, customer focused and knowledgeable point of contact. To provide a consistent approach across the Trust, Standard Operating Procedures (SOP) will describe how the functions of this role will be carried out along with the timescales that will be adhered.
