The post involves closely working with the Geriatrician in OPAU clinic. The Older Person Assessment Unit is led by a part-time consultant Geriatrician in addition to an elderly Registrar who rotates in conducting Continence clinic in the acute. The post also rotates in Continence Advisory Service across Lambeth & Southwark clinics, alongside contributing to the clinical governance framework of the service.

The role will play a key role in forging links with the Home delivery services to provide appropriate products for in-patients and out-patients and they will maintain a close working relationship with, and support colleagues in Primary Health Care Teams.

The post holder will conduct high-quality nurse-led continence advice clinics through either face to face, video, or telephone consultations providing expertise in a range of bladder and bowel problems.
The nurse specialist role will deliver specialist continence care within elderly care medicine and surgery in line with national recommendations across both hospital and community sites. The post holder will act as an expert practitioner, resource and facilitator to educate and train other staff in the acute & community including students as well as hold the responsibility for managing a caseload of patients with complex bladder and/or bowel problems who require specialist assessment and treatment.

The post holder will be responsible for ensuring a rapid response service is provided to the wards in relation to bladder and bowel issues and delayed discharges.

The post holder will be responsible for contributing to the ongoing development of clinical practice, research and standards of care within the service, including the development of policies, protocols and guidelines and contributing to service developments in collaboration with multidisciplinary colleagues.

Also ensures the safety and well-being of service users, and always works in a manner that promotes dignity and human rights through the adoption of person-centred care principles.

Professional / Clinical responsibilities
• To be professionally and legally responsible for all aspects of the post-holders work.
• To carry out an assessment of patients as an autonomous practitioner, including complex presentations, and using clinical reasoning skills and knowledge of evidence-based practice, determines an appropriate care plan utilising a wide range of treatment skills and options.
• To interpret and analyse clinical and non-clinical facts to form accurate diagnoses in a wide range of conditions including those with a complex presentation, with occasional guidance from specialist nurses.
• To formulate accurate prognoses and recommend the best course of the intervention, developing comprehensive discharge plans.
• To continually reassess patients in order to progress treatments effectively.
• To coordinate intervention which may include other disciplines: advises and educates patient/carers/relatives/other health professionals.
• To manage clinical risk within the caseload at all times.
• To demonstrate physical ability to carry out specialist assessment and interventions including manual therapy techniques and therapeutic handling.
• To demonstrate highly developed dexterity, coordination and palpatory sensory skills for assessment and manual treatment of patients.
• To be able to manage potentially stressful, upsetting or emotional situations in an empathetic manner, including but not limited to those in Continence Advisory Service.

Management and Leadership
• To be responsible for supervision and coordination of junior staff/students and assistants on a daily basis.
• To be responsible for the operational management of the Band 6 nurse in the absence of the team leader or as required.
• To flexibly plan and organise own time, patient caseload, meetings, training and adjusting plans as required according to staff.
• To carry an identified bleep as required and be able to respond to unpredictable work patterns and frequent interruptions.
• To assist in planning juniors, students and assistants' timetables and deputise for the team leader in organisational duties.
• Organise team meetings at an appropriate frequency, recording and circulating minutes of these.
• To ensure a good method of communication within the department at all times and to encourage the exchange and implementation of new ideas and evidence-based practice.
• Provide cover for the other Clinical Nurse Specialist as appropriate.
• Implement and work within the Trust’s HR policies on all staffing matters, including sickness, capability and disciplinary issues. Take corrective measures if indicated in liaison with Human Resources and the Head of Nursing.
• Provide a leadership role model and demonstrate expert knowledge with high standards of clinical practice
• Develop the role according to patients changing needs and research findings, within the resources available.
• Adhere to Trust and departmental policies and procedures, ensuring that all staff does the same.

Communication
• To be able to motivate and persuade others through effective communication skills, with the benefit of verbal and non-verbal skills, using written and electronic information where needed.
• To demonstrate the ability to communicate complex and sensitive information to patients, carers, and other staff, where there may be barriers to communication i.e. non-English speaking patients; use of interpreters; excess noise or lack of privacy.
• To maintain a close link, communication and liaison between all staff and people involved in patient care, student education, and research or policy development as appropriate.

Information management
• To maintain an accurate and evaluate record-keeping system: Electronic Patient Referrals for own clinical caseload
• To supervise junior and student nurses record-keeping system according to departmental standards.

Education & Professional Development
• Act as a facilitator/mentor/preceptor to student nurses and others.
• Assess your own educational needs and take steps to keep up to date with clinical and nursing developments in line with the requirements of the Nursing & Midwifery Council.
• Develop the partnership between the Trust and academic staff at local Universities involved in pre-registration and post-registration nursing and medical programmes.
• Ensure that the learning environment and specialist training are appropriate for both pre and post-registration learners and participate in the development of appropriate programmes.
• Work closely with the Sister/Charge Nurse, and Practice Development Teams within the area, ensuring that the relevant clinical-based teaching programmes are in place for all staff.
• Contribute to the Trust’s clinical supervision programme for the staff
• Teach relevant courses and training programmes provided by the Florence Nightingale School of Nursing & Midwifery/KCL and within the area. To lecture on external courses as required.
• Lead and participate in education and training programmes within the specialty service, and develop training packages as required.
• Coordinate the development and delivery of a patient-focused education and information-giving strategy within the specialty.
• Motivate, develop, support and identify training needs for the department staff.
• Participate in appraisals and setting of objectives for junior staff in the department to enable them to achieve optimal effectiveness and to understand their contribution to the directorate.
• Ensure the appropriate written induction programmes are available and utilised for all new staff within the department.

Research and Audit
• To participate in the department’s ongoing audit and appropriate work-related research/evaluation projects.
• Utilise research findings in the delivery of specialist patient care, developing new ways of working and disseminating relevant information to staff.
• To keep abreast of evidence-based practice in the clinical area by use of relevant reading, attendance at in-service training, external courses and database searches.
• To ensure a good working knowledge of national and local standards and monitor quality as appropriate.
• To be familiar with current clinical research protocol issues and relevant treatment and procedures employed in the diagnosis and treatment within their specialty.
