Summary
An exciting full-time Band 7 HIV Community Clinical Nurse Specialist (CNS) job opportunity has arisen in HIV outpatient services provided by Homerton Healthcare. This is a brilliant opportunity for someone looking for a full time role in which they can make a difference to some of the most vulnerable People Living with HIV in our care. You'll be supported by an experienced team of specialist doctors and nurses, within the unit.
The HIV Community CNS provides holistic care to a caseload of our more vulnerable People Living with HIV (PLWHIV), their friends and families within the socially and culturally diverse inner city borough of City and Hackney.
You will be expected to work autonomously as part of an extensive and supportive multi-disciplinary team to deliver person-centred care to a caseload of PLWHIV who have multiple and complex health and social care needs.
The role involves supporting patients to engage in healthcare and includes home visits, adherence support, phlebotomy, care coordination, advocacy and liaison with other health and care stakeholders.
You will have advanced clinical skills in assessing the needs of PLWHIV and will be working towards completion of the Advanced Physical Assessment course. You will have experience of safeguarding vulnerable adults and young people and have a solution-focused approach to problem solving.
