Summary


Staffordshire and Stoke-on-Trent Integrated Care Board (ICB) was created on 1 July 2022 and is responsible for planning and commissioning high quality health services for the people of Staffordshire and Stoke-on-Trent.

We have strong clinical leadership and work as part of a complex system of health and social care, which includes Local Authorities, Providers, the Independent Sector, Staffordshire Training Hub and GP practices.

We are looking for a Practice Nurse Facilitator to join the team to work alongside three other Practice Nurse Facilitators across the ICB

An exciting opportunity has arisen for an experienced highly motivated individual to join the Primary Care Team working across the Staffordshire and Stoke-on-Trent Clinical Commissioning Groups.

This is an exciting and innovative role, for a forward-thinking individual who will provide professional advice and leadership to General Practice Nurses incorporating clinical service change and improvement. A key focus will be on strengthening the nursing workforce by identification of educational needs and providing access to training and development opportunities needed to implement high quality and evidence-based practice. The role will act as a conduit between Practice Nurses, Nurse Associates, pre-registration nurses and Health Care Assistants. We are looking for a nurse who is highly motivated, enthusiastic and enjoys a challenge. In addition, you must hold current registration and have an understanding of the role of the nurse in Primary Care.

A key focus will be on strengthening the nursing workforce by identification of educational needs and providing access to training and development opportunities working closely with the Staffordshire Training Hub, to implement high quality and evidence-based practice.

The Staffordshire and Stoke-on-Trent NHS Integrated Care Board (ICB)is responsible for planning functions; managing the NHS budget and arranging the provision of health services in the geographical area.

Location Home based/hybrid working (most of posts).

Travel on occasions to a Hub location with at least 1 days notice

New starters are expected to collect IT equipment, from offices in Stoke-on-Trent, and if based at home are required to have appropriate desk/chair/ relevant equipment in compliance with DSE regulations

The role of a Practice Nurse Facilitator is a pivotal role in strengthening the nursing workforce in primary care
to ensure continuous learning and development opportunities to support the delivery of high quality patient
care.

The role will be to assist in improving the quality of primary care in line with the ICB primary care strategy. The role will act as a conduit between Practice Nurses, Nurse Associates (including trainees), preregistration nurses and HCAs’ roles; and the ICB and wider including the ICS, Staffordshire Training Hub and Health Education England.
